Prostate Cancer Screening: PSA Test Baseline


Dr. David Samadi clarifies confusion surrounding the new government recommendation that healthy men forgo PSA screening. As he explains, the panel’s decision is based on older studies of older men, rather than newer studies of younger men – the group most likely to benefit from PSA tests. Since 1990, prostate cancer deaths have decreased 40% as a result of early prostate cancer testing and early diagnosis. Dr. Samadi stresses the need for individualized care explaining that testing healthy, younger men allows them to establish a PSA baseline, from which annual PSA screenings can monitor changes referred to as the PSA velocity.

Tim Glynn on his PSA Experience


Tim Glynn discusses his experience with the PSA screening and what patients should know before deciding to have the screening. Tim Glynn, a lawyer from Setauket, NY, was 47 years old in 1997 when he went to his primary-care doctor, troubled by a vague feeling of being down. After his physical exam, Glynn was sent to have his blood drawn. Along with thyroid and cholesterol level testing, the doctor ordered a PSA test. A week later, Glynn returned to hear the results. His PSA was elevated. He was told to get a biopsy as soon as possible. This is his personal story as to what he believes patients should know about the PSA test. He is not a medical professional and his opinions are his own. The Foundation for Informed Medical Decision Making believes that PSA screening should be made in the context of a shared medical decision. Shared medical decision making allows patients and their providers to make treatment decisions together, taking into the account the best scientific evidence available as well as the patients’ values and preferences.

New Study On Prostate Cancer Screening Effectiveness


Dr. David Samadi, member of the Fox News Medical “A-Team” and Vice Chair of Urology at Mt. Sinai Medical Center in New York, discusses his thoughts on a new study in the British Medical Journal that states that there is no benefit to prostate cancer screening. Dr. Samadi feels that there is a definite benefit to screening, and that the data used for this study is faulty due to the lack of standardized PSA testing during its first six years. He also places emphasis on doctors looking at several factors that help determine whether a patient is at risk of prostate cancer. This includes not only PSA level, but watching PSA velocity (the rate at which PSA levels rise over time), and paying attention to a patient’s family history, as there is a strong genetic link to prostate cancer developing. When caught early, prostate cancer has a very high cure rate. However, if it is not treated and it spreads to the bone then treatment becomes incredibly difficult. This is why Dr. Samadi feels that carefully monitoring each patient on an individual basis and paying attention to the risk factors involved in each case, along with PSA screening, is an incredibly beneficial process.
